Digital Announcement Request Form

1. Please provide all pertinent details of your announcement, along with any specific requests for how your information is shared with the congregation.
2. Please specify “where” you would like your announcement displayed (i.e., Pre-service Sanctuary slides, Narthex TV slides, End of Service—Pastoral Announcements, clcs.org Website, Social Media or the Outdoor Digital Sign).
3. Please upload any pertinent files for display or to assist in creating your digital announcement (e.g., picture, logo, video clip, PPT slide). If more than three files are needed, submit additional files by completing a second request form.

Please submit your request and accompanying materials by 12:00 PM on the Wednesday prior to the weekend the announcement is to be displayed.
